Before jumping into whether Rainbow Six Extraction all operators MIA is a possibility, you need to understand the MIA system. Extraction includes a brilliant system that allows death and damage to carry over from match to match. Whenever an operator is dealt any amount of damage, it will remain there for the next match, encouraging you to move onto another operator while they heal. Since operators are removed from the playable roster when left in an incursion zone, does that mean that all operators can go MIA leaving you with no one to play? The short answer is no. While the operators will be unplayable when MIA, they have a cooldown before they return, which is usually within a few matches. The consequence of this is that your operator will face a 30% reduction in their overall XP, which can be especially crushing when you are closing in on the max rank of 10. This will remove stat boosts like 20% reduction in incoming damage and so on. It can also reduce your overall level, and reductions can be as big as an entire levels as you get into the higher difficulties. 

So by that count, you will likely only be able to have around 3-4 operators MIA at any given time, and that is if you are failing one mission after the other. While this can appear rather limited when you are in the first stage of the game, by the time you reach Milestone 17, you will have 18 operators to choose from.
