To get Diablo Immortal Fading Embers, you will need to complete Elder Rifts that are enhanced by Crests. Crests come in both Rare and Legendary, and can be applied to any rift up to three times by a single player to increase the rewards, and guarantee items like Runes and Gems. 

Using a Rare Crest will get you one Fading Ember, while a Legendary Crest nets you a total of five. All four players in the Rift can also stack their Crests together, giving rewards to everyone, so the optimal way of maxing your Ember acquisition is by teaming with others that have Crests to use. This means you can make a total of 60 Fading Embers in each Rift if all the players use Legendary Crests.
